The high-yield points, in one place.

  • Left = Lungs. Left-sided failure backs up into the lungs (crackles, dyspnea). Right-sided backs up into the body (edema, JVD).
  • Digoxin plus low potassium is dangerous. A potassium under 3.5 raises toxicity risk even at a normal dig level. Check potassium and apical pulse first.
  • MI priorities: oxygen, ECG, pain relief. A worsening rhythm or a dropping blood pressure outranks a stable abnormal value.
  • Check the number before beta blockers and ACE inhibitors. Hold or question them when the BP or heart rate is already low.
  • Two right answers means it is testing priority. Airway, breathing, circulation, and the least stable client come first.

Why B is correct: pink, frothy sputum with worsening dyspnea signals acute pulmonary edema, a life-threatening complication of left-sided failure that needs immediate intervention. Ankle edema and JVD are signs of right-sided failure and are expected findings, not emergencies. A 1 kg gain over a week is worth noting but is not the priority over a client who is acutely decompensating. Sit the client upright, apply oxygen, and notify the provider.

Why B is correct: hypokalemia (below 3.5 mEq/L) increases the risk of digoxin toxicity, which can cause life-threatening dysrhythmias, even when the digoxin level itself is within range. The nurse holds the dose and notifies the provider so the potassium can be corrected first. Giving the dose, altering it, or adding fluids does not address the underlying electrolyte problem and a nurse does not change a prescribed dose independently.

Why A, C, D are correct: beta blockers slow the heart rate, so clients check the pulse and hold the dose for bradycardia. They cause orthostatic hypotension, so slow position changes prevent falls. They can trigger bronchospasm, so new dyspnea or wheezing must be reported. Why B and E are wrong: beta blockers are never stopped abruptly because rebound hypertension and angina can follow, and they blunt the heart-rate response to exercise rather than raising it.
